A thoughtful and responsive referral process.

Crossroads to Change Residential Treatment Center partners with DFPS Placement Teams, SSCC Placement Coordinators, juvenile probation departments, case managers, caregivers, hospitals, crisis units, schools, and community partners to support young men in need of structure, stabilization, and therapeutic care.

Every referral is reviewed individually by our team to ensure appropriate placement fit, safety, and support needs, with timely communication throughout the admissions process.

How admissions work.

Submit Referral Information

Referral sources submit youth information for review. Our team will evaluate whether Crossroads to Change can safely and appropriately meet the youth's needs.

Initial Review

Our admissions team reviews available records — behavioral history, clinical needs, education status, medication information, and placement goals.

Required Documents

Please provide available clinical, medical, education, and placement / DFPS documents (full checklist below).

Admissions Decision

Once documents are reviewed, Crossroads to Change notifies the referral source regarding acceptance, denial, or need for additional information.

Step 3 · Required documents

Documents we'll need at intake.

At Crossroads to Change Residential Treatment Center, please provide whatever is available. Missing documents won't disqualify a referral — they help us serve the youth safely from day one.

Clinical / Behavioral Health

  • Current psychological or psychiatric evaluation
  • Recent therapy notes (if available)
  • Psychiatric medication monitoring notes
  • Hospitalization records from the last 6 months (if applicable)
  • CANS assessment (if applicable)

Medical

  • Current medication list
  • Medication administration records
  • Allergies and medical conditions
  • Recent physical or well-child documentation

Education

  • Most recent report card
  • ARD / IEP (if applicable)
  • School withdrawal paperwork (if applicable)
  • School behavior or incident reports

Placement / DFPS

  • Common Application
  • Service Plan
  • Legal status documentation
  • Placement history
  • Any safety plans or court orders
